Account Registration Data: Full name, email address, phone number, practice name and address, NPI number, medical specialty, and state license information. NPI numbers are verified against the NPPES National Plan and Provider Enumeration System operated by the U.S. Centers for Medicare & Medicaid Services.

Order and Transaction Data: Billing address, shipping address, order history, product selections, and payment method type (we do not store full credit card numbers; payment processing is handled by PCI-DSS compliant processors).

Usage Data: Pages visited, features used, session duration, device type, browser type, and IP address. This information is collected automatically and used to improve platform performance and security.

Communications: Records of support requests, messages sent through our contact form, and any correspondence with our account executive team.

Personal Information you provide: name, email, billing/shipping address, phone, institutional details, payment information (processed securely by third parties), order messages.

Automatically Collected:

• IP address, device/browser info, usage data (pages visited, timestamps)

3. Information Sharing and Disclosure

Service Providers: We share information with third-party vendors who assist in operating our platform, including cloud hosting providers, payment processors, shipping carriers, and email delivery services. These providers are contractually obligated to use your information only as directed by Life Peptides.

Legal Requirements: We may disclose information when required by law, regulation, subpoena, court order, or governmental authority. We will notify you of such requests to the extent permitted by law.

Business Transfers: In the event of a merger, acquisition, or sale of all or a portion of our assets, your information may be transferred as part of that transaction. We do not share your NPI number, prescribing information, or patient-related data with pharmaceutical manufacturers, data brokers, or any advertising networks.

4. Data Retention

We retain personal information for as long as your account is active or as needed to provide services. We also retain information as necessary to comply with legal obligations, resolve disputes, prevent fraud, and enforce our agreements.

Order records and transaction data are retained for a minimum of seven (7) years to comply with applicable financial and tax regulations.

NPI verification records are retained for the duration of your account and for five (5) years following account closure.

You may request deletion of your account and associated personal data at any time by contacting support@lifepeptides.com. Certain data may be retained after deletion where required by law.

5. Security

We implement industry-standard technical and organizational measures to protect your personal information, including:

• TLS/SSL encryption for all data in transit
• AES-256 encryption for sensitive data at rest
• Role-based access controls limiting staff access to personal data
